CARE Unit
The CARE Unit is a team of highly trained and experienced registered nurses and case managers.

Caregiver & Dementia Support Services
We provide information and assistance for grandparents and other relatives age 55 and older who are caring for children.

Friendly Visitor & Telephone Reassurance Programs
Volunteers provide friendly visits and telephone check-ins for the homebound on a schedule agreeable to both client and volunteer.

Health Promotion
We provide assistance accessing a wide variety of opportunities including evidence-based chronic disease self-management and falls prevention programs, Senior Center programming, exercise and wellness opportunities, and support groups.

Medicare & Health Insurance Counseling
Trained staff and volunteers help with Medicare, Medicare Advantage, prescription drug, EPIC, supplemental and long-term care policy decisions and problems.

Housing
We provide assistance obtaining subsidized housing, help with energy bills and weatherization, home repairs, and home modifications to increase the ease of use, safety, security and independence in the home.

Legal Assistance
We provide information, assistance, referrals and services to ensure the rights of older adults, including prevention of abuse and neglect, assistance with consumer fraud and scams, and assistance with advance directives (wills, health care proxies and powers of attorney).

Nutrition Program
The Eat Well Live Well Program congregate meal program promotes better health through nutritious lunches, socialization and supportive services provided at sites through the county.

NY Connects
NY Connects is endorsed and supported by the New York State Office for the Aging in collaboration with the New York State Department of Health.

Personal Emergency Response Systems
We provide information and services to help select and access in-home 24-hour electronic devices for summoning help in an emergency.
